Child Must Be 5 By September 1st

Dates for registration: Tuesday, March 12th,  Thursday,  April 11th, and Tuesday, June 4th

Registration Time: 8 am-4 pm

Location for registration: Chattooga’s Enrollment Center 286 Maple Dr. Summerville Ga

What to bring: 
Proof of residence (Utility bill, Rent/lease agreement, Notarized Residence Affidavit)
Driver’s License or picture ID of parent/guardian that is enrolling the child.
Child’s Birth Certificate
Child’s Social Security Card Proof of Immunization (3231) Eye, Ear, Dental Form (3300) Custody papers when applicable
